Interweave is a digital health platform enabling NHS Trusts, Social Care organisations, Local Authorities, Ambulance Services, and GP Practices to access real-time health and care information across providers and IT systems. Designed and built by the Yorkshire and Humber Care Record as part of NHS England's Local Health and Care Record Exemplars programme, the platform uses an open-source federated model with no data lake and integrates with existing clinical systems. A partnership model means organisations jointly own the system without vendor lock-in.
